COMPLETE THE FORM AND SUBMIT TO THE ROTARY SERVICE GROUP VIA E-MAIL. YOU WILL BE CALLED WITH A CONFIRMATION NUMBER (RA#) TO BE ATTACHED TO THE UNIT BEFORE SHIPPING TO HAAS.

IMPORTANT NOTICE - Please review our new Rotary Return Authorization (RA) policy below. The change being made is significant. Please insure that everyone at your organization who is involved with rotary repairs is informed of the change.

Effective the 1st of November 2006 we will require a purchase order up to $1000.00 when the RA number is issued or a credit card number. We will call when the repairs are complete to advise of the cost. Should the cost of the repair be more than $1000.00 we will call and ask for approval of the repair costs. Should the cost of the repairs exceed the customers expectations we will return the unit in the state that it was received. Return shipping will be best way ground unless otherwise requested.

We will not bill your credit card until the repairs have been completed and have advised you of the cost.

The intent of this new policy is to reduce the turn around time for the repair of Haas rotaries products. During the past couple of years, we have experienced many long delays in shipping completed repairs back to customers. These delays are mostly due to problems in arranging for the payment of repairs (RA’s).

In the past year, the average cost of repairs was about $1000.00. We are asking that a "not to exceed" purchase order for $1000.00 be faxed to Haas at the time the RA number is issued and if possible a copy included with the RA that is shipped to Haas Automation Inc. for repair.

Limited Warranty Coverage

After the completion of a rotary repair, a 90-day service and parts warranty applies.

Factory-specified maintenance procedures must be adhered to and recorded in order to maintain this warranty.

This warranty is void if the rotary is subject to mishandling, misuse, neglect, accident, improper installation, improper maintenance, or improper operation or application, or if the machine was improperly repaired or serviced by the customer.
